Customer Response Form Script
The Customer Response Form script allows your business to obtain specific information from a customer. This could be useful for mailing information packages, informing them of new products, getting their comments or suggestions, etc. This script prompts the customer to complete an online form. Then the information is e-mailed to you and a confirmation e-mail is sent to them. This is an example of our customer response form used for gathering keywords and descriptions about our client’s Web sites. This example is taken from ImagiNET's Search Engine Submission Form.

1. The customer enters the required information. Once finished the customer clicks the SUBMIT button to process the form.

2. Once the form is processed, a thank you confirmation page is displayed on the screen.

3. Then an e-mail message is sent to the business containing the information entered by the customer.

4. Also, a confirmation e-mail message is sent to the customer for their records.
